New Volunteers: Print all forms and return as hard copies to the Site Supervisor. NOTE: fingerprinting is mandatory by Vine. It takes a few days for the prints to be processed. Please set an appointment to have your fingers printed at your earliest convenience. If you have any questions regarding fingerprinting, contact Vine @ 408-655-2080.
Returning Volunteers: You only need to complete the following forms if you need to make any changes to the forms we currently have on file.
