New Students

To apply to Vine Academy please fill out this online form:

Student Application 

After completing the application, each item listed below needs to be completed and submitted to Vine online or by mail (address located at the bottom of the application)

  1. Student Application
  2. Additional Student Application (Sibling Application)
  3. Teacher Reference
  4. Pastoral Reference
  5. Adult Reference

Once the above items have been received, an interview will be scheduled.

If you have received an acceptance letter from the board, print each of the following forms, fill out and return by mail to Vine Academy: 777 1st St. PMB 101, Gilroy, CA 95020

  1. Student Emergency Form (must have original copy)
  2. Vine Release Form (must have original copy)
  3. Waiver / Release Form (must have original copy)
